Universal Service Fund (E-rate information)

E-rate information for NetworkVirginia customers.

Thanks to Greg Weisiger (Greg.Weisiger@doe.virginia.gov) of the Virginia Department of Education for providing the following information regarding E-Rate for NetworkVirginia participants.

Network Virginia is a state master contract negotiated by Virginia Tech, Old Dominion University and the Community College system in 1996.

The contract expired in 2005 and was renewed in accordance with a law enacted by the General Assembly. Because the contract extension was done without competition, there was no associated Form 470 for the extension.

Consequently, beginning with Fund Year 2005, schools and libraries had to use their own 470s to use the Network Virginia contract.

The contract is a two-part contract with Verizon providing broadband backbone and point-to-point service and Sprint generally providing Internet access. Pricing information is available at the Network Virginia Web site:

http://www.networkvirginia.net/netva. Near the bottom of the page is ordering information. Submission of this form and receipt acknowledgment will be proof of a "contract" for E-Rate review.http://www.universalservice.org/sl/applicants/step07/form471-attachments.aspx

Item 22: Usually the Block 4 Worksheet for the school, school division or library system Item 23: Pricing will depend on services requested. Please consult your account representative to verify pricing and confirm any one-time installation charges (E-Rate eligible).

The Sprint Block 5 will be the same as the Verizon Block 5, using the

following:

Sprint's SPIN is 143005695

Sprint Contract Number is CC-0696-SPRINT Contract award date is the date of the memorializing memo for existing customers OR the actual contract signature date for new customers Contract expiration date is 06/13/2008. Note this date is different from an email sent two years ago and reflects actual language on the contract addendum. Please note that the Contract Expiration Date is just over two weeks before the end of the fund year. If you used 2010 on Year 2006 or Year 2005 forms, use 2008 this year.

If you want to receive funding for the last 17 days of June under the Sprint side of the Network Virginia contract you will need to do an additional Block 5 using your 2007 Form 470 number (this year), and request a single month of service as MTM, rather than a contract.

Because this is MTM rather than contracted service, you will check YES in Box 15a and put 06/14/2008 as the service start date.
